How to Request a Formal Assessment

 

Please READ ALL INSTRUCTIONS and make your request early in the Calendar Year. 

This will allow us time to find and schedule the appropriate Ref Coach. 


1. Send an e-mail to the State Director of Assessments, Dale Berry: dberry@daje2.com  requesting an assessment. Indicate whether it is for maintenance or upgrade. 

(EX: Regional Maintenance OR Upgrade from Grassroots to Regional)


Minimum Information Required

-League of Play

-Assignor

- Game Date

-Time

-Location

- Team Names

- Names & Position of Referee Crew

-Your Referee ID Number (Found on Learning Center)

-Your Contact Information (Cell, Email, etc)


2. If requesting an upgrade, attach a list of games officiated. This will help determine if the appropriate number of games have been officiated at the appropriate age group and level. (Click the link at the bottom of this page to access the game count template.)


3. Please confirm how the assessment will be paid for as well as who to send the money to prior to the day of the assessment. Best practice is to request this information in your original email to Dale Berry.


Regional Upgrade Match Fee:

Grassroots to Regional = $150/ Game (3 games as referee (2:adult/ 1:U-19))

Regional Maintenance Match Fee: 

Regional = $150/ Game (2 passing assessments)

National Upgrade Match Fee:

Regional to National = $200/ Game (3 games at Selected Track)

National Maintenance Match Fee:

Regional to National = $200/ Game (2 games at Selected Track)


4.  Contact your assessor within 48 hours of match to verify game information.


5. Most assessments will be planned in advance and everyone properly notified. Field evaluations may be unplanned. It is expected that each game will be officiated as if an assessor were present. 


6. Assessments and Feedback will be due to the paying Official within 10 days of the Match (if in-person) or upon receiving the Match Film. 

(Extenuating circumstances may present and the SRC must be notified.)
